First Visit
Your First Visit for Oral and Maxillofacial Surgery
Your first visit to our office will consist of a comprehensive examination and consultation to develop a personalized treatment plan tailored to meet your individual needs. It is important that any patient under the age of 18 be accompanied by the parent/guardian.
During this exam, your doctor will perform a visual exam, as well as review your current X-rays. If necessary, additional X-rays may be taken to ensure efficient treatment planning.
Upon completion of the exam, a detailed treatment plan will be presented to you. We will thoroughly explain the details of the recommended procedure and answer any questions you may have.
Afterwards, we will schedule the appointment(s) for the procedure and make financial arrangements.
Please assist us by providing the following information at the time of your first visit:
- A surgical referral form completed by your general dentist/orthodontist.
- Any recently taken radiographs.
- A complete list of current medications you are taking or are allergic to.
- Medical and dental insurance card(s).
- Completed Registration Forms
